New Patients
Please ask at the Reception desk for forms to be able to register with Penygraig Surgery.
You will need to supply :
Your NHS number ( available form your current GP practice )
Identification confirmation ( Photo ID / Driving licence / Passport )
Address confirmation ( Utility Bill / Postal addressed letter / rent or mortgage agreement )
A copy of the request form for any repeat medication you are currently being prescribed.
If Newly registered patients are currently on a Hospital waiting YOU ARE ADVISED to contact the hospital and advise them you are/have changed GP practice, this will ensure clinical information is sent to the correct practice without delay.
Practice Boundary
Penygraig Surgery provides general medical services for patients in the following:
- Penygraig
- Penrhiwfer
- Tonypandy
- Trealaw
- Williamstown
All Newly registered patients will be required to complete a patient registration/health questionnaire and an acceptable behaviour contract. Without these forms completed your registration will be delayed.
If you are a unpaid Carer Please remember to inform the practice. The practice has a Carers Champion who may be able to signpost you to relevant help agencies.
